RIDGEVIEW Behavioral Health Services, a not-for-profit community mental health center headquartered in Oak Ridge, TN, has an immediate opening for a Full Time Bachelor’s Level TOC (Transitions of Care) Care Coordinator/ Data Analyst to provide intensive case management to individuals who have high utilization of emergency rooms and inpatient services in the Anderson County community.  This will often be fast paced, with each day presenting new opportunities and challenges as you respond to the needs of individuals actively receiving care.  This position is unique in that it will also provide data gathering and analysis for Ridgeview's work in this area.

REQUIREMENTS

Bachelor's degree.  If Bachelor's isn't in social sciences or mental health (ex counseling, psychology, social work, vocational rehabilitation) or includes 15+ credit hours of related courses on transcript, then relevant training and supervision will be provided after hire.  Prefer experience working with individuals with severe and persistent mental illness.  Must maintain a valid TN driver's license and a safe driving record.  The position requires a self-starter with excellent time management skills.

JOB DUTIES

Care coordination and case management refers to a process for directing and coordinating services to consumers of mental health services.  The Care Coordinator works directly with adult or youth clients, their families, and community resources both in and out of Ridgeview to secure necessary services to meet client needs.  This position will focus on the Transitions of Care (TOC) portion of the Tennessee Health Link (THL) program with an emphasis on individuals who have high utilization of emergency rooms and inpatient services.  This position will also provide data gathering and analysis for TOC.
