Ridgeview Behavioral Health Services, a not-for-profit community mental health center, based in Oak Ridge, TN, has an immediate opening for a full-time TN Health Link Adult Care Coordinator based out of LaFollette location (Campbell County).

REQUIREMENTS

A Bachelor's degree in the social sciences (psychology, sociology, counseling, mental health, etc); two years of professional experience working with the SPMI population preferred; safe driving record is required.

JOB DUTIES

Care coordination is a process for directing and coordinating services to consumers of mental health services.  It is a client-centered, goal-directed, and recovery-oriented method for identifying needs and securing services so that a full array of appropriate and necessary services and assistance in accordance with the individual's overall plan of service and current behavioral health standards.  Clients will be provided intensive case management services, including follow-through admissions, treatment, and discharge procedures for all targeted consumers.
